        一直说既往急性加重是最重要的急性加重预测因子,那么为什么会发生这些急性加重?首先,要鉴别是否真的是急性加重,由于是回顾性判断,很难区别仅仅是一次呼吸道感染,还是感染诱发了急性加重,还是其他诱因所致的急性加重,或者根本是肺外问题导致的呼吸系统症状加重(比如心血管疾病)等等;其次,急性加重的原因,是下呼吸道致病微生物负荷升高,是气道炎症水平升高,是胃食管返流等合并症没有控制好,还是药物没有好好用?等等。不同的原因应该有不同的应对方式,ICS一招鲜显然有点过于简单了,有时甚至南辕北辙。
