Outcomes of Vesicoureteral Reflux in Children with Non-neurogenic Lower Urinary Tract Dysfunction with Targeted Treatment at their Specific LUT Condition
Abstract
Purpose
There is a known association between non-neurogenic LUT conditions and VUR. Whether VUR is secondary to the LUT condition or coincidental is controversial. We sought to determine our rate of VUR resolution in patients with LUT dysfunction using TT for their underlying condition.
Materials and Methods
Patients diagnosed and treated for a LUT condition who had concomitant VUR at or near the time of diagnosis were included. Patients were managed with TT and antibiotic prophylaxis and their VUR was monitored with VCUG or VUDS.
Results
VUR was identified in 58 ureters in 41 patients (36 F, 5M; mean age: 6.2 yr). After mean 3.1 years of treatment, VUR resolved with TT in 26 of 58 (45%) ureters. All had a history of UTIs prior to starting TT. Resolution rates of VUR were similar for all VUR grades. Resolution or significant improvement of VUR was higher in the ureters of patients with dysfunctional voiding (70%) compared to those with idiopathic detrusor overactivity disorder (38%) or detrusor underutilization (40%).
Conclusions
VUR associated with LUT conditions resolved with TT and antibiotic prophylaxis in 45%. Unlike the resolution rates reported in VUR without a coexisting LUT condition, we found that there were no differences in the resolution rate of VUR between grades I through V in patients with LUT conditions. Patients with dysfunctional voiding had most improvement and resolution of their VUR. Additionally, grade V reflux resolved in some patients.
