Contraceptive implants were similarly effective in obese, overweight, and normal-weight women.
Most U.S. women of reproductive age are overweight, but company-sponsored clinical trials of contraceptive products often have excluded women whose body weight is >130% of ideal. To establish whether overweight and obesity reduce the effectiveness of contraceptive implants, investigators analyzed data from a prospective cohort that included 1168 U.S. women who chose to use the etonogestrel contraceptive implant and 4200 who chose an intrauterine device (IUD; either the levonorgestrel intrauterine system or copper T380A). Among implant users, 28% were overweight and 35% were obese; weight distribution among IUD users was similar.
During 1377 woman-years of implant use, one woman (who was obese) experienced an unintended pregnancy. Cumulative implant failure rates were 0.23 per 100 woman-years for obese participants and 0.00 per 100 woman-years in both normal and overweight groups. During 5985 woman-years of IUD use, overall failure rates were <1.00 per 100 woman-years and did not vary by body-mass index (BMI) status. No significant differences were detected in the effectiveness of implants compared with IUDs in any BMI category.
